Below are forms available for you to fill out prior to your appointment.
NEW PATIENTS: Acupuncture: A non-refundable initial payment of $120.00 due at time of scheduling appointment is required to hold your appointment. FootBaths: non-refundable fee ($35 for only 1 or half down for packages) Iridology: non-refundable fee $65.00 deposit. Pay the difference time of appointment.
Please be advised this office does notaccept insurance. We are not providers for any insurance company. If your insurance company claims to cover Acupuncture you may submit to your insurance for reimbursement.
ALL PATIENTS: Appointments cancelled less than 24 hours notice will be charged the full cost of the session. Monday appointments, cancellations must be made by 2 pm Friday.
Important: If you are NOT filling out paperwork ahead of time and are doing so the day of your appointment please arrive 30 minutes prior to avoid running into your treatment time. I start on time. Thank you
